Annual Benefit Open Enrollment for Active Employees and Retirees on Conventional Plans.
The Human Resources Department will accept benefit forms via Email**, by US Mail, Fax at 508-799-1040 or in-person. No applications will be accepted after 4:30 p.m. May 16, 2025.
We also encourage you to submit inquires via Email or telephone.
This is the one time each year that active employees and retirees on conventional plans can make any changes to Health, Dental or Vision Insurance.
If you are an active employee, this is your one time to make changes to your voluntary products:
- Term Life Insurance
- Universal Life Insurance
- Short and/or Long Term Disability
- Medical and Dependent Care Spending Accounts.
- Health Savings Account
Are you adding coverage for the first time or adding someone new to your plans? Marriage certificates, birth certificates, divorce decrees, social security numbers and primary care physicians are required at the time of enrollment. We cannot process without ALL required documentation.
**IMPORTANT: To safeguard your information, please omit any personally identifiable information (PII) such as social security number(s), when submitting documentation by email. A Human Resources customer service representative will contact you to confirm your submission by telephone.
The FY26 Open Enrollment is a PASSIVE enrollment. Other than participation in the Flexible Spending Accounts (FSA) and/or Health Savings Account (HSA) a PASSIVE enrollment means employees are automatically re-enrolled in the benefits based on their selection made in the past. An enrollment election is required to continue in the flexible spending accounts (FSA - Visit CPA125.com) & the health savings account (HSA).
