Diverse Business Directory Entry Questionnaire

This form is to add your business listing to the Diversity Directory hosted by the City of Worcester. The purpose of this directory is to highlight which businesses are diversely-owned, so that other small businesses, larger businesses, customers or potential clients who wish to work with or support these businesses can connect with each other.

The questionnaire is broken into three sections. The first section (Business Information) and second section (Owner Information) contain questions that will be listed in the public Diverse Business Directory. Questions with an asterisk (*) are required; all others are optional, but will help improve the content of our directory. The questions in the third section (Additional Information) are for our internal use only and will not be made public.
