The City of Worcester Department of Health and Human Services seeks a dynamic professional to serve at the Community Navigator. Reporting to the Commissioner of Health and Human Services or their designee, the Community Navigator will connect with homeless individuals through street outreach, support individuals through their reentry process from behind the Worcester House of Corrections, and navigate referral processes for housing and wraparound services. This position serves as a liaison between community organizations and the homeless population & incarcerated individuals. The Community Navigator will provide transportation for clients for wraparound service appointments and will hold weekly office hours to connect with clients.

The successful candidate must have a strong understanding of the social determinants of health and health equity and a strong understanding and experience working with vulnerable populations. The qualified applicant will have the ability to maintain records and prepare reports; to conceptualize and organize projects into manageable tasks; to collaborate and work as part of a team; and to prioritize tasks. Additionally, the Community Navigator will have excellent time management skills and be proficient working with electronic databases.

MINIMUM REQUIREMENTS
• Bachelor's Degree in public health, health and human services, or a related field
• Three to five (3-5) years' experience working with vulnerable populations
• Valid driver’s license and reliable transportation

PREFERRED QUALIFICATIONS
• Master's Degree in public health or a related field
• Five to seven (5-7) years' experience working with vulnerable populations
• Experience working at a local health or human services agency or at a local nonprofit agency

SALARY: $25.67 - $28.53, hourly, part-time, nineteen (19) hours/week
