Body Art & Piercing

Any person seeking a permit to practice body art in the City of Worcester must complete an application package. A practitioner must also have a valid CPR certificate, a valid First Aid certificate, a valid Blood Borne Pathogens certificate and a skin course.

First Aid and CPR Certifications will no longer be accepted if completed online. Classes must be taken in person.

We encourage you to submit your application ONLINE through our permit portal link below! Please complete all required fields, electronically sign and upload the appropriate documents.

  1. Infection Control Policy - Consent Form and After Care Information
  2. Certificate of Compliance with Worcester Revised Ordinance Regarding Revenue Collection
  3. Certificate of Compliance with the Workers' Compensation Act (NOTE: Please Notarize)

Application Requirements:

  1. Current Blood Bourne Pathogens Certification
  2. Current CPR Certification
  3. Current First Aid Certification
  4. Current Microblading Course Certification
  5. Proof of Basic Anatomy and Physiology Course Completion
  6. Proof of Hepatitis B Vaccination
  7. Proof of Skin Course Completion
  8. Proof of one year apprenticeship under the supervision of trained, experienced piercer (if new applicant)

Temporary License (Event Based) • Apply Now!

  • Temporary Establishment for an Event
  • Temporary/Visiting Practitioner

Please Note: In an effort to support the department's initiative to go paperless, there will be a $50 Administrative Fee added to all permit and license applications that are submitted over the counter or by mail effective January 1, 2022. We encourage you to take advantage of the online permitting & license portal links above where you can apply for all permits/licenses without an added Administrative Fee.

You may also mail your application to the Housing/Health Division, however we recommend you follow up to ensure that it was received and that only copies of the original documents are mailed.
