Forms Information

You may order a Certificate of Municipal Lien (CML) by printing the CML Request Form (36KB). Fill it out and return it to: City Treasurer's Office, Room 203, 455 Main Street, Worcester, MA 01608, Attn: CML. Please fill out a separate form for each parcel (map/block/lot) requested. You must also include payment, in the correct amount, per certificate with your request.

You can also order a CML ONLINE through Worcester's online payment partner.

We will complete the certificate and return it to you within 10 business days of the receipt of your request. If you have any questions on ordering a CML, please call us at (508) 799-1075 ext. 1087 or email treasurer@worcesterma.gov.

You may order a Betterment Release by sending a written request. The request should be accompanied by a copy of title examination and must include the book and page number of the document you wish to have released. Please include your name, the property address, and the parcel ID with your request and payment information to City Treasurer's Office, Room 203, City Hall, 455 Main Street, Worcester, MA 01608. You must also include a payment of $4.00 per certificate with your request.

To file a claim for injury or property damages against the City of Worcester, you must print out the Claim Form, fill it out, sign it and return it to the City Clerk's Office, Room 206, City Hall, 455 Main Street, Worcester, MA 01608.
